In order to view the files, you will need Adobe Acrobat Reader. If you do not have Adobe Acrobat Reader, you can download the latest version from the Adobe web site, free of charge.
Once you have printed an application packet from the internet, please remember to read all instructions very carefully, then fill in the forms neatly and completely. Completed forms and payment need to be mailed to ICD.
