ICD Teacher Credentials Evaluation Application

Please remember to read all instructions very carefully, then fill in the forms neatly and completely. Once all forms are completed:

1. Send the completed Application (pages 3–7), all enclosures and your payment to: International Consultants of Delaware, Inc., 3600 Market Street, Suite 450, Philadelphia, PA 19104-2651 USA.
2. Forward all “Request for Academic Records” forms to the appropriate educational institution(s).
   NOTE: Applicants must have received their ICD ID number before requesting documents from institutions or license authorities. Documents without ID numbers will not be processed.
3. Forward all “Request for Validation of Registration/License” forms to the appropriate registration authority(ies).
   NOTE: Applicants must have received their ICD ID number before requesting documents from institutions or license authorities. Documents without ID numbers will not be processed.
