Please remember to read all instructions very carefully, then fill in the forms neatly and completely. Once all forms are completed:
- Send the completed application, items requested in the enclosure checklist and your payment to: International Consultants of Delaware, 3600 Market Street, Suite 450, Philadelphia, PA 19104-2651 USA.
- Forward all Request for Academic Records/Transcripts for Credentials Evaluation forms to the appropriate educational institution(s). Applicants must have received their ICD ID number before requesting documents from institutions or registration/license authorities. Documents without ID numbers will not be processed.
