General Statement Credentials Evaluation — This report provides general information and comparability statements regarding the education you earned outside the United States. It identifies the institutions where you studied, attendance dates and the U.S. equivalency of all degrees/diplomas/certificates earned. Courses and credits are indicated.

Course-by-Course Credentials Evaluation — This report contains all of the information that a General Statement Credentials Evaluation provides, and also includes information regarding the educational institutions where you studied, your attendance dates, as well as providing a detailed breakdown with the U.S. comparability of the courses, credits and grades and the degrees/diplomas/certificates you earned outside the United States. The Course-by-Course Credentials Evaluation also offers two optional services:

Grade Point Average Service — Using a U.S. grading conversion system, this service provides the U.S. comparability and an average of the grades you obtained in each subject studied outside the United States.

Summary Breakdown Service — This service provides a breakdown listing of four different types of required science courses: biological sciences, chemistry, mathematics and physical sciences.

Re-evaluation — The re-evaluation is for the purpose of assessing and/or evaluating additional credits to make up deficiencies indicated in a previous evaluation. It must be initiated prior to the three-year time limit for requiring a new application.

Additional services

Additional report recipient — This option allows you to select additional report recipients beyond the two who already receive your report as part of the basic application.

Overnight courier (domestic/international) — This service provides overnight delivery of a completed report to your designated recipient(s). The fee must be paid for each recipient that you designate.

Originals returned by courier — This option allows you to have any original documents you submitted as part of your application returned to you by courier.

Document translation — If any of your documents are not in English and you were unable to provide a certified word-for-word translation, ICD can arrange for document translation for an additional fee. See number 5 of step 2 on page 2 of your application, called Enclosure checklist, for translation instructions.

Full payment for all services requested must be included with your application. Send only a certified bank check or international money order, drawn in U.S. dollars on a U.S. bank, and made payable to “ICD” or pay be credit card using the Credit Card Payment Form included on your application. Personal checks and cash are not accepted.
