
ICD Physical Therapy Assistant Credentials Evaluation
This evaluation is used for applying for state board licensure in the United States and/or for further education, employment and certification in support of an immigration application. The Physical Therapy Assistant Credentials Evaluation contains:
- time period(s) of study,
- institution(s) attended,
- all degrees/diplomas/certificates earned,
- U.S. comparability of the international educational credentials,
- whether or not one is licensed to practice as a physical therapist in the country of education,
- total educational credits awarded,
- source of the educational credentials (transcripts/diplomas/degrees/certificates) submitted directly to ICD, and
- state board form listing categories/courses/credits for specific state boards of physical therapy.

Please remember to read all instructions very carefully, then fill in the forms neatly and completely. Once all forms are completed:
- Send the completed application, items requested in the enclosure checklist and your payment to: International Consultants of Delaware, 3600 Market Street, Suite 450, Philadelphia, PA 19104-2651 USA.
- Forward all Request for Academic Records/Transcripts for Credentials Evaluation forms to the appropriate educational institution(s).
- Forward all Request for Validation of Registration/License for Credentials Evaluation forms to the appropriate registration authority(ies).
NOTE: You must have received your ICD file number before requesting documents from institutions or registration/license authorities. Documents without file numbers will not be processed.

Once ICD has received your application, you will be emailed a document checklist that lists any addtional required documents. This email also includes your ICD file number. If a report has not been issued within 12 months of the date the first document checklist is sent to you, the application expires. If you want to continue with the evaluation, you need to apply to reprocess your expired application and pay the associated fee.
