ICD Physical Therapy and Physical Therapy Assistant Credentials Evaluations for applicants educated outside the United States
This evaluation is used for applying for state board licensure in the United States. The Physical Therapy and Physical Therapy Assistant Credentials Evaluations contain:
- time period(s) of study,
- institution(s) attended,
- all degrees/diplomas/certificates earned,
- U.S. equivalent of the international educational credentials,
- whether or not one is licensed to practice as a physical therapist in the country of education,
- total educational credits awarded,
- source of the educational credentials (transcripts/diplomas/degrees/certificates) submitted directly to ICD, and
- state board form listing categories/courses/credits for specific state boards of physical therapy.
