This evaluation is used for applying for state board licensure in the United States. The Physical Therapy Credentials Evaluation contains:

time period(s) of study,
institution(s) attended,
all degrees/diplomas/certificates earned,
U.S. equivalent of the international educational credentials,
whether or not one is licensed to practice as a physical therapist in the country of education,
total educational credits awarded,
source of the educational credentials (transcripts/diplomas/degrees/certificates) submitted directly to ICD, and
state board form listing categories/courses/credits for specific state boards of physical therapy.
