Required Documents

After you have selected the credentials evaluation service you would like to apply to, you will need to read the appropriate section below to determine the required documents you will need to obtain to submit as part of your ICD application. Each application packet contains forms for obtaining the appropriate educational and licensure documents (Request for Academic Records and Request for Validation of Registration/License).

For a downloaded Application: Complete the top portion of the document request form(s), and send the form(s) to the schools or licensing authorities, who need to then complete the bottom portion. The school(s) and licensing authority(ies) are then responsible to send the forms directly to ICD.

For an application that was mailed to you: Complete the front portion of the document request form(s), and sends the form(s) to the schools or licensing authorities, who need to then complete the back portion. The school(s) and licensing authority(ies) are then responsible to send the forms directly to ICD.

For a General Credentials Evaluation

Required documents for a General Credentials Evaluation application are country-specific. to find out what documents are required in your country, please call +1 215-222-8454 ext 510 between the hours of 9am to Noon, U.S. Eastern Time, Monday through Friday.

For a Physical Therapy Credentials Evaluation

All (post-secondary) educational and licensure documents must be OFFICIAL documents sent to ICD directly from the source institution/agency. The following documents must accompany your ICD application:

Secondary educational documents: You may send original language, notarized photocopies of all diplomas, degrees or certificates earned during your secondary school education. However, external exam results will not be accepted if submitted by yourself. If you do not have a copy of an official secondary school credential(s), you need to request that the issuing/granting agency verify their external examination results and/or date of graduation by writing directly to ICD.

Post-secondary educational documents: You must complete the applicant portion of the request form(s) and send them to the appropriate educational institution(s). The registrar's office needs to complete the school's portion of the document request form and, in addition, send official academic records, which should include transcripts or marksheets with courses, grades, credits/contact hours, course description/syllabi and proof of conferral of degree/diploma/certificate.

Registration/licensure documents: You must complete the applicant portion of the request form(s) and send them to the appropriate issuing authority(ies). The registration authority(ies) needs to complete the agency's portion of the document request form and, in addition, send offical validation of the applicant's certificate of registration/license. You must provide official validation of ALL professional registrations/licenses you hold, including initial and all current registrations/licenses.

Name change: You must provide documentation of any legal name change, if applicable (for example, a Marriage Certificate).

Certified English translations of any documents not in English: The following sentence must be typed or written at the end of the translation: “Certification of Authenticity: This is to certify that this is a true and correct English translation of the attached photocopy of the original [name of document] of [your name].” This sentence must then be signed by the translator. Note: ICD reserves the right to verify any English translation provided as part of an application packet.

For a Certified Public Accountant Credentials Evaluation
