Documents that are Required/Accepted to Verify Identity:

ICD has forms for these identity docs, which have to be notarized and will be collected after you purchase a service, as your application processes.

Driver’s license with photo

After you have selected an ICD credentials evaluation service, you will need to read the appropriate section below to determine the required documents you will need to obtain to submit as part of your ICD application. Each application contains forms to obtain the appropriate educational and licensure documents (Request for Academic Records/Transcripts for Credentials Evaluation and Request for Validation of Registration/License for Credentials Evaluation).

Complete the top portion of the document request form(s) and send the form(s) to the schools and/or licensing authorities, who will then complete the bottom portion. The school(s) and licensing authority(ies) must send the forms directly to ICD.

For All Evaluations

All post-secondary (tertiary) educational and licensure documents must be OFFICIAL documents sent directly to ICD from the institution/agency. The following documents must accompany your ICD application:

Secondary education documents: You may send original language, notarized* photocopies of all diplomas, degrees or certificates earned during your secondary school education. You can notarize your documents online here. However, external exam results will not be accepted if submitted by yourself. If you do not have a copy of an official secondary school credential(s), you need to request that the issuing authority send official exam results and/or date of graduation directly to ICD.

Post-secondary (tertiary) educational documents: You must complete the applicant portion of the request form and send it to the appropriate educational institution. The school authority needs to complete its portion of the document request form and, in addition, send your official academic records/transcripts, which should include marksheets with courses, grades, credits/contact hours, course description/syllabi and proof of conferral of degree/diploma/certificate. If needed, print additional copies of the form to send to other schools. Additionally, we require notarized photocopies of all post-secondary diplomas, degrees, or certificates earned during your post-secondary school education. Registration/licensure documents**: You must complete the applicant portion of the request form and send it to the appropriate issuing authority. The issuing authority needs to complete its portion of the document request form and/or send official validation of your certificate of registration/license. Official validation of ALL professional registrations/licenses you hold, including initial and all current registrations/licenses, must be provided. If needed, print additional copies of the form to send to other issuing authorities.

Name change: You must provide documentation of any legal name change, if applicable (for example, a marriage certificate).

Certified English translations of any documents not in English: Any documents that have not been issued in English must have an English translation performed by a certified translator. The following sentence must be included at the end of every translation: “Certification of Translation – I certify that I am fluent in English and (language of document), and that this translation is true and accurate.” The translator must sign each translation. You CANNOT translate your own documents.

ICD relies on the accuracy of English translations, so it is in your best interest to be sure the translation is accurate. ICD cannot be responsible for evaluations that are based on inaccurate English translations. The current fee for translations is $85 per page; this fee is subject to change without notice.
