Required Documents

Please refer to your Application Status Tracker on your ICD portal for the remaining documents needed for your evaluation.

Identity Documents

Two different forms of government issued notarized identity are required for all evaluation types. All identity documents must be active (not expired).

Acceptable forms of identity include, but are not limited to:

  • Driver’s License
  • Passport / Passport Card
  • Birth Certificate
  • Marriage Certificate
  • Legal Name Change Affidavit

Unacceptable forms of identity include but are not limited to:

  • Library cards
  • Work IDs
  • School IDs

Notarization may be done by a local Notary in your area, or you may utilize the online services of NotaryCam if you would like: icdeval.com/notarycam

Please note that you must submit documentation of any legal name change.

Upper Secondary Education Documents

Diplomas/degrees/certificates earned during upper secondary education may be notarized and uploaded to the portal by yourself. If you do not have a copy of your official upper secondary school credentials, the issuing authority must send a copy directly to ICD. These can be sent through physical mail to the below address:

Post-Secondary/Tertiary Education Documents

Transcripts
Transcripts must be sent from your educational institution directly to ICD, and include courses taken, grades, and credits/hours for each course. These cannot be uploaded by you.

Curriculum/Course Descriptions
Curriculum/Course Descriptions must be sent from your educational institution directly to ICD; these cannot be uploaded by you. Curriculum/Course Descriptions are needed for every course that appears on your transcript. Educational institutions most commonly submit official documents that contain (but are not limited to):

  • A description of the course
  • Learning objectives
  • Syllabi
  • Required Readings

Education Form
The Education Form can be found on your portal and must be filled out and sent from your educational institution directly to ICD. This cannot be uploaded by you.

Diploma/Degree/Certificate of Credential Earned
Diplomas/degrees/certificates earned during post-secondary/tertiary education may be notarized and uploaded to the portal by yourself. Alternatively, your educational institution may also send your diploma/degree/certificate directly to ICD along with your other documents. If sent by your educational institution, it does not need to be notarized.

Supervised Training Documents

The Supervised Training Form can be found on your portal and must be filled out and sent from the organization where the supervised training took place, directly to ICD. This cannot be uploaded by you.

License/Registration Documents

The License/Registration Form can be found on your portal and must be filled out and sent from the organization that issued your license/registration, directly to ICD. This cannot be uploaded by you.

Language Proficiency Scores

ICD collects International English Language Testing System (IELTS) scores. You will be asked to enter your Test Report Form (TRF) number upon completing your profile information if these scores are needed from you. Your TRF number is the 15–18 digit number mentioned in the bottom right-hand corner of your Test Report Form document.

Certified English Translations

Any documents not issued in English must be accompanied by an English translation performed by a certified translator. You may NOT translate your own documents. ICD relies on the accuracy of English translations for evaluations and ICD cannot be held responsible for evaluations that are based on inaccurate English translations.

Please refer to the below list for the methods of translation accepted by ICD:

  • Translations may be coordinated through ICD.
    • The current translation price is $85/page
  • Translations may be performed by your educational institution and submitted directly to ICD.
  • Applicants educated in China may have translations coordinated through Beijing China Health International Education Consulting Co. LTD

ICD reserves the right to verify any English translation provided with an application.
